School Nurse Staff

Each school in our district has a licensed nurse assigned to support medical needs.  For a printable list by school, see SCHOOL Medical Staff  below.

The Diabetes Medical Management Plan (document provided, completed and signed by the treating physician) is required along with the two documents below.

A BCSS Medication Permit Form and one of the following school specific forms are required for any daily prescription medication administered at school (ADD, ADHD, etc.)

• Physician Diagnosis & Medication Dosage Verification Forms

• BCSS Med Permit

If your child is injured and requires the use of crutches and/or a wheel chair at school, you must have the following form completed and signed by a physician upon returning to school

• BCSS Physician's Documentation for Use of Medical Equipment for Mobility at School
